Start by scheduling your initial telehealth visit with a licensed EverPhase provider.

Once booked, you’ll receive secure access to:

    •    Your private patient portal

    •    Comprehensive intake forms

    •    Required consent documents

This allows your provider to fully understand your health history, symptoms, goals, and priorities before your visit — so your time together is focused and productive.

All care is provided via secure telehealth for your convenience.

Prior to your visit, you’ll complete a detailed intake covering:

    •    Medical history

    •    Current medications and supplements

    •    Lifestyle factors (nutrition, sleep, stress, exercise)

    •    Symptoms and health goals

If you’ve had labs completed within the last 6 months, your provider can often use those.

If not, Labs and specialty tests are ordered and managed through our secure Fullscript platform — patients receive emailed instructions, a digital requisition or shipped test kits, and can track their labs online

Step 3: Your Personalized Care Plan

During your consultation, your provider will:

    •    Review your intake and any lab results

    •    Identify root causes and contributing factors

    •    Explain treatment options clearly

    •    Collaborate with you on a realistic, personalized plan

Your care plan may include lifestyle guidance, supplements recommendations, and treatment options. Labs and specialty tests are ordered and managed through our secure Fullscript platform — patients receive emailed instructions, a digital requisition or shipped test kits, and can track their labs online
